Description
The Division of Cardiology at Albany Medical College is seeking a full-time Nurse Practitioner or Physician Assistant to join our specialized Heart Failure outpatient practice. This position offers the opportunity to provide high-quality, patient-centered care in a collaborative environment while working closely with heart failure cardiologists and a multidisciplinary care team.
This role will primarily focus on the outpatient management of patients with heart failure, supporting continuity of care across the disease spectrum. Comprehensive training in heart failure management and ventricular assist devices (VAD) will be provided.
Position Highlights
- Full-time, Monday–Friday day schedule
- Outpatient clinic-based role
- Collaborative practice with heart failure cardiologists and multidisciplinary team members

Responsibilities
- Conduct outpatient visits for patients with heart failure, including ongoing management and follow-up care
- Perform comprehensive patient assessments, including detailed medical histories and physical examinations
- Order and interpret diagnostic studies such as laboratory tests and cardiac imaging
- Develop and implement treatment plans in collaboration with supervising physicians
- Provide medication management, patient education, and counseling on disease management and lifestyle modifications
- Coordinate care with primary care providers and other specialists
- Respond to patient inquiries related to ongoing care
- Document patient encounters, treatment plans, and coding accurately in the electronic health record
- Support clinic workflow and patient throughput as part of a team-based care model
Qualifications
- Current license and active registration as a Nurse Practitioner or Physician Assistant in New York State
- Master’s degree from an accredited Nurse Practitioner program or completion of an accredited Physician Assistant program
- Interest in or experience with cardiovascular or heart failure care preferred
- Familiarity with electronic medical records; Epic experience preferred
- Strong communication and collaboration skills
- Commitment to patient-centered care and maintaining HIPAA confidentiality standards
